The Patient Care Coordinator performs a wide variety of duties associated with patient services.. Coordinates initial patient contact and ensures patient eligibility to clinic and verifies that the appropriate primary care physician is listed prior to setting up appointments. Initiates Comfort Care appointments for patients as needed and trouble shoots problems that may arise between Comfort Care and the patient. Coordinates with the physicians and ensures a timely submission of the attestation sheets to the Data Rap department if applicable. The HCW will collaborate with staff and other clinical departments within and outside the MTF to facilitate excellent patient care and will be expected to participate fully as a member of the 99 MDG medical staff. Attend and participate in meetings, professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, flight meetings, and other meetings required by applicable regulations, MTF guidance, or as directed by the Flight Commander or Medical Director. Familiarization with Department of Defense (DoD), Federal, State, non- profit healthcare systems and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is highly desired. Productivity will meet current Medical Group Management Association (MGMA) standards for Nellis Air Force Base. The MGMA standard is updated annually on the fiscal year. Shall possess sufficient knowledge of current TRICARE standards, population-based health care, disease management, medically-oriented computer systems, and trends in prevention.
Draw actionable conclusions based on analyses performed, make recommendations through use of healthcare analytics, predictive modeling, and communicate those conclusions effectively to audiences at various levels of the enterprise. Experience with building dashboards in Excel, Power BI, and/or Tableau and data management. Knowledge of healthcare operations (utilization management, disease management, HEDIS quality measures, claims processing, etc.). Demonstrated understanding of key managed care concepts and provider reimbursement principles such as risk adjustment, capitation, FFS (Fee-for-Service), Diagnosis Related Groups (DRG’s), Ambulatory Patient Groups (APG’s), Ambulatory Payment Classifications (APC’s), and other payment mechanisms.. Experience with Databricks and TOAD Data Point
Provides administrative and clerical support for the clinic/department including patient scheduling, filing reports, and following up on documentation, phone calls, and billing inquiries.. Certified Medical Assistant (CMA) or Registered Medical Assist (RMA) required.. Certified by one of the listed governing associations required: NHA, AMT, AAMA, NAHP, NCCT, AAH, or AMCA. Job Duties Obtains and documents, using the rooming tool in the electronic medical record, the patient’s reason for visit, weight, vital signs, visit screening information, medication review, and pertinent information for provider.. Under the direction of the provider and/or site lead, assists with orientation of new personnel.. Follows hand hygiene and appropriate infection prevention precautions.
Manage 4-6 patients per hour in Soldier Readiness Processing (SRP) and 2-3 patients per hour in Separation Health Physical Examination (SHPE).. Order and review laboratory tests, X-rays, and other diagnostic reports, following up on abnormal results within one week.. Adhere to MTF productivity standards, infection control policies, and universal precautions as per CDC guidelines.. Abide by all facility regulations, Joint Commission standards, and professional clinical protocols in accordance with MTF policies.. Basic Life Support (BLS) Certification from the American Red Cross or American Heart Association, maintained in a current status.
