The Patient Care Coordinator performs a wide variety of duties associated with patient services.. Coordinates initial patient contact and ensures patient eligibility to clinic and verifies that the appropriate primary care physician is listed prior to setting up appointments. Initiates Comfort Care appointments for patients as needed and trouble shoots problems that may arise between Comfort Care and the patient. Coordinates with the physicians and ensures a timely submission of the attestation sheets to the Data Rap department if applicable. The HCW will collaborate with staff and other clinical departments within and outside the MTF to facilitate excellent patient care and will be expected to participate fully as a member of the 99 MDG medical staff. Attend and participate in meetings, professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, flight meetings, and other meetings required by applicable regulations, MTF guidance, or as directed by the Flight Commander or Medical Director. Familiarization with Department of Defense (DoD), Federal, State, non- profit healthcare systems and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, Medicare, Medicaid and the Veterans Administration (VA) is highly desired. Productivity will meet current Medical Group Management Association (MGMA) standards for Nellis Air Force Base. The MGMA standard is updated annually on the fiscal year. Shall possess sufficient knowledge of current TRICARE standards, population-based health care, disease management, medically-oriented computer systems, and trends in prevention.
