Follows a Protocol approved by the Providers of prescribed treatment and therapies within the scope of the RN. Meets with the patient for a total of 6 sessions. Based on the RN assessment the patient will be scheduled for an appointment with their PCP if appropriate. Provides support to the Providers and/or patients participating in the ICT Program.
