Licensed Practical Nurse – Skilled Nursing & Rehab

Ontario Center seeks a compassionate Licensed Practical Nurse to provide hands-on care in our skilled nursing and rehab facility. The LPN will deliver medications and treatments, monitor resident status, and support restorative therapies in a resident-centered environment. Responsibilities include assisting with ADLs, documenting care, collaborating with an interdisciplinary team, and promoting safety, dignity, and comfort. Provide direct nursing care to residents including medication administration, treatments, and monitoring of vital signs Support physicians and RNs by following care plans and reporting changes in resident condition Assist residents with daily living activities while promoting dignity, safety, and comfort Document care, assessments, and interventions accurately in medical records and electronic systems Collaborate with interdisciplinary team members to coordinate individualized care and rehab goals Educate residents and families on care plans, treatments, and health maintenance Participate in care conferences, quality improvement, and regulatory compliance activities Maintain infection control, safety, and confidentiality standards at all times Supervise and support CNAs and other support staff in daily care tasks Respond to emergencies and follow facility protocols for acute changes in condition Medication administration Wound care IV therapy (where permitted) Vital signs monitoring Electronic medical records (EMR) documentation Infection control practices Geriatric nursing care Rehabilitation and restorative care Care planning support Patient and family education Active LPN license in NY BLS/CPR certification Long-term care or rehabilitation experience preferred IV certification where required by facility policy
