Hospice Registered Nurse Job at Heartland Hospice in Nekoosa

Heartland Hospice is seeking a compassionate Hospice Registered Nurse to provide skilled, patient-centered care to individuals with life-limiting illnesses in home and facility settings. The RN will conduct comprehensive assessments, manage pain and symptoms, administer medications and treatments, and coordinate individualized care plans. Collaborating closely with an interdisciplinary team, the nurse will educate and support patients and families, ensure continuity of care, document accurately, and uphold regulatory and quality standards while honoring dignity, comfort, and patient goals at end of life. Provide skilled hospice nursing care in home and facility settings Perform comprehensive patient assessments and ongoing evaluations Develop, implement, and update individualized care plans Manage pain and symptoms, administering medications and treatments Educate and support patients and families on disease process and end-of-life care Coordinate care and communicate with the interdisciplinary hospice team Document assessments, interventions, and outcomes accurately and timely Ensure compliance with hospice regulations, policies, and quality standards Participate in on-call rotation as needed to support patient needs Advocate for patient comfort, dignity, and goals of care Hospice nursing care Pain and symptom management IV therapy and medication administration Wound care Care plan development Electronic medical record (EMR) documentation Patient and family education Interdisciplinary team collaboration Regulatory and compliance knowledge (hospice) Crisis intervention and de-escalation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license in state of practice BLS certification Hospice and Palliative Nurses (CHPN) certification preferred Valid driver’s license and reliable transportation Experience in hospice, palliative care, oncology, or home health preferred
