Schedule: Mon - Fri 8am-4:30pm, 40 hrs/wk. No call.
Required:
Active/clear CT RN license (or compact), Associates or Bachelors degree. Prior hospice nursing experience. Experience with Homecare Homebase (HCHB). Strong case management and interdisciplinary team collaboration skills. Ability to manage a hospice caseload independently while providing compassionate, patient-centered care.
