Director, SI&S Community Impact & Patient Reach

Responsibilities: develop and implement models to expand reach identify barriers via community insights and evidence build enterprise tools and frameworks for impact measurement foster cross-sector collaboration to drive sustainable community health initiatives

Requirements: Bachelor's degree in Public Health, Healthcare, or related field 10+ years in public health, social impact, or health equity deep understanding of health disparities, social determinants, and patient access barriers proven experience with data analysis, strategy development, and scaling community programs

Preferred: advanced degree (e.g., MPH, MBA, PhD) experience with multi-stakeholder coalitions impact evaluation health ecosystems High-Value:focus on health equity in diverse populations community health social determinants cross-functional stakeholder engagement Work Setup:hybrid at East Hanover (3x/week); no visa sponsorship or relocation support.
