Occupational Therapy Assistant Job Duties:
- Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
- Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress with the use of innovative EMR software.
- Occupational Therapy Assistants employed by Flagstar Rehab will receive: Fully Paid Orientation.

Occupational Therapy Assistant Requirements:
- Current NYS OTA License and RegistrationEMR Competence
- State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
- Valid Proof of Identity and Work Authorization
- Malpractice/Liability Insurance (if applicable)
