Home Health Registered Nurse Case Manager

AccentCare is seeking a Registered Nurse / Patient Care Manager for our home health team serving Hanford, CA. In this role, you will deliver and coordinate high-quality in-home nursing care for patients receiving pharmaceutical therapies, reflecting our biotechnology and pharmaceutical focus. You will complete comprehensive assessments, develop and adjust individualized care plans, and oversee a caseload to ensure safe, effective treatment. Working within a collaborative, innovation-driven culture, you’ll partner with physicians, pharmacists, and therapists, contribute to quality improvement, and mentor clinicians while maintaining rigorous safety and documentation standards. Provide skilled nursing care to home health patients, including assessments and ongoing evaluation of clinical status. Develop, implement, and update individualized patient care plans based on physician orders and clinical standards. Coordinate interdisciplinary care with physicians, therapists, pharmacists, and caregivers to optimize outcomes. Manage a caseload of home health patients, prioritizing visits, documentation, and follow-up needs. Administer medications, IV therapies, and wound care while adhering to safety and infection-control protocols. Educate patients and families on medications, disease management, and home care techniques to support independence. Document all assessments, interventions, and outcomes accurately in the electronic health record (EHR) in a timely manner. Monitor clinical quality indicators, report changes in condition promptly, and participate in case conferences. Ensure compliance with regulatory, accreditation, and company policies for home health services. Mentor and support field clinicians, contributing to a culture of collaboration, learning, and continuous improvement.
