RN - Manager

Registered Nurse - Home Health: ScheduleFlexible scheduling based on patient needs.

Assignment Details: Full-time or part-time positions available; guaranteed hours may vary.

Requirements: Must be a graduate of an approved school of professional nursing. Active RN license in the state of employment. Minimum of two years nursing experience and one year experience in home health. Experience with OASIS and HomeCare HomeBase required. Valid state driver's license and automobile liability insurance.

Responsibilities: Individualize and execute the client's plan of care; reassess as necessary. Ensure appropriate documentation is completed for all patients transferred to an inpatient facility. Process and verify documentation for OASIS and 485s. Lead weekly case conferences to discuss patient care and discharge decisions. Contact physicians to obtain necessary orders for service provision. Document and follow up on vital sign alerts and wound score deviations. Participate in on-call rotation as needed.
