Key Responsibilities: Conduct thorough evaluations of patients, reviewing their medical history and performing specific assessments to identify individual needs and goals. Develop and communicate comprehensive treatment plans and goals to patients and their families, ensuring they are involved in the recovery process. Implement treatment interventions based on recent evidence-based practices and the unique needs of the patients we serve, adapting protocols as necessary for each individual. Work collaboratively with a multidisciplinary team of healthcare providers, documenting and addressing any changes in patient conditions in accordance with treatment goals. Maintain precise and timely documentation of all patient care activities to ensure compliance with regulatory requirements and contribute to departmental objectives. Pursue opportunities for continuous professional development and education, actively seeking out role as a clinical instructor for less experienced therapists. Patient Population / Client FocusAs a Home Care Physical Therapist, you will work with a diverse range of clients, including elderly individuals recovering from surgery, patients with chronic pain conditions, those with neurological disorders, and individuals undergoing rehabilitation after hospitalization. Our focus emphasizes empowering patients by providing them with the tools and support to achieve their recovery goals in the comfort of their own homes, thus enhancing their engagement and overall satisfaction with care.

Qualifications: A Bachelor's Degree in Physical Therapy is required. While we emphasize experience, candidates with a Doctorate in Physical Therapy (DPT) will be prioritized. A valid DPT license in the State of New Jersey is mandatory. Current certification in Basic Life Support (BLS) as provided by the American Heart Association is required. While a minimum of 2 years of experience is expected, recent graduates passionate about providing quality care will also be considered.

Preferred Experience: Experience in home care or outpatient settings is preferred, particularly familiarity with working with geriatric populations and implementing therapy in a home-based environment. Previous exposure to electronic health record (EHR) systems will be an advantage for maintaining efficient documentation and coordination of care.

Compensation and Benefits: The salary range for this position is between USD 100,000 and 120,000 annually, commensurate with experience and education.
