This is a 13-week, full-time contract designed for a clinician who values autonomy and the chance to provide essential care in patients homes.
Schedule: Full-time, Monday through FridayOn-call responsibility probable
Responsibilities include:
- Delivering high-quality, in-home nursing care and skilled interventions to diverse patients
- Conducting comprehensive assessments and completing OASIS documentation accurately and promptly
- Collaborating with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Providing essential services such as wound care, medication management, disease monitoring, and patient/family education
- Communicating observations and updates promptly to ensure optimal patient outcomes
- Managing a structured schedule with a set number of daily visits
Qualifications:
- Active RN license
- At least 1 year of recent experience as a Home Health RNProven proficiency with OASIS documentation (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Ability to work independently and thrive in a fast-paced, schedule-driven environment
