Physical Therapist

NOTE: This position requires US Citizenship.

SUMMARY: The Home Visit Physical Therapist Assistant provides rehabilitative care under the supervision of a licensed Physical Therapist, following the patient's individualized Plan of Treatment.

RESPONSIBILITIES: This role supports clinical collaboration, integrates adaptive equipment to enhance outcomes, and communicates patient progress to the care team. The PTA ensures accurate documentation per APTA and regulatory standards, participates in quality initiatives, and coordinates follow-up services. Commitment to confidentiality, professional development, and timely billing documentation are essential.

REQUIREMENTS: Home Health experience; HCHB preferred. Work provided in-home in Jefferson, Oldham, Henry, Shelby, and Trimble counties.
