Hospice Bereavement Navigator

The Community Hospice in Albany is seeking a Bereavement Service Facilitator to join Trinity Health as part of our hospice care team. The role runs 9am–5pm, three days a week, serving patients and families in the Capital Region as part of our Interdisciplinary Team. The position requires an Associate Degree in Human Services and 2 years of experience in a human services setting; a Bachelor's in Human Services is preferred.
