MAÉ Health is seeking a Care Coordination Specialist to conduct comprehensive member needs assessments and coordinate care plans, referrals, and follow-up activities with payer care management teams. You will build trusted relationships with members through compassionate, culturally responsive communication and outreach by phone, text, and email, while maintaining accurate documentation and escalating high-risk concerns when needed.
