The ENT Physician Minimum Qualifications:- MD/DO from an accredited medical program
- Physician Licensed or eligible for license to practice medicine in Illinois
- Board certified or board eligible in the appropriate specialty(ies)
- Licensed by the Federal Drug Enforcement Administration
- General ENT physician that is also trained in allergy
