Licensed Practical Nurse – Long-Term Care

Signature HealthCARE seeks a compassionate Licensed Practical Nurse to deliver resident-centered care in our long-term care and post-acute community. The LPN will administer medications and treatments, monitor resident conditions, document in EHR, and collaborate with an interdisciplinary team to support individualized care plans. Provide direct resident care under RN and physician direction Administer medications and treatments per orders and facility policy Monitor, document, and report changes in resident condition Perform wound care, dressing changes, and basic restorative care Collaborate with interdisciplinary team on care plans Educate residents and families on care, treatments, and medications Maintain accurate, timely EHR documentation Uphold infection control, safety, and quality standards Support resident-centered routines and dignity in daily care Precept and support new team members as requested Medication administration Wound care Vital signs monitoring Electronic Health Records (EHR) documentation IV therapy (where permitted) Care planning support Infection control Patient assessment Catheter and ostomy care Communication with interdisciplinary teams Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN/LVN license in state of practice BLS/CPR certification Experience in long-term care, rehab, or skilled nursing preferred Familiarity with EHR systems Knowledge of infection prevention and control standards
