Hospice and Home Health On-Call Registered Nurse

Gentiva is seeking a compassionate Registered Nurse On Call to provide home-based care for hospice and home health patients during evenings, nights, weekends, and holidays. You will triage calls, make urgent home visits, assess changing conditions, and implement care plans that support comfort, dignity, and safety. Collaborating with an interdisciplinary team, you will provide symptom management, patient and family education, and timely documentation. Provide on-call home visits for hospice and home health patients during evenings, nights, weekends, and holidays Triage patient and family calls, assess urgency, and determine appropriate interventions Perform comprehensive nursing assessments and implement care plans focused on comfort, safety, and dignity Administer medications, manage symptoms, and perform necessary treatments and procedures Collaborate with physicians, hospice, and home health team members to coordinate ongoing care Educate patients and families on disease processes, medications, and home care instructions Document assessments, interventions, and communications accurately and promptly in the EMR Participate in hand-offs with day staff to ensure continuity of care and accurate follow-up Adhere to all clinical, safety, infection control, and regulatory standards in home settings Support Gentiva’s mission-driven, compassionate culture in every patient and family interaction Triage and clinical assessment IV therapy and medication administration Wound and pain management Care planning and coordination Hospice and palliative care knowledge EMR documentation Patient and family education Infection control practices Driving and route planning Time and caseload management Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN license in state of practice Current BLS certification Valid driver’s license and reliable transportation Home health or hospice experience preferred Electronic medical record (EMR) proficiency
