Licensed Practical Nurse – Long-Term Care & Rehabilitation

Life Care Centers is seeking a compassionate LPN Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you’ll provide direct, patient-first nursing care to seniors in a rehabilitation and long-term care setting. Responsibilities include administering medications, monitoring vital signs, assisting with treatments, and accurately documenting care. You’ll collaborate with an interdisciplinary team to support individualized care plans, educate residents and families, and promote excellent healthcare outcomes in a family-focused environment that encourages professional growth. Provide direct nursing care to residents in a senior living and rehabilitation setting Administer medications and treatments according to physicians’ orders and facility policies Monitor, document, and report changes in resident condition, including vital signs and symptoms Support development and implementation of individualized care plans with the interdisciplinary team Maintain accurate, timely documentation in electronic health records Promote patient safety and infection control standards at all times Assist residents with daily living activities as needed, ensuring dignity and comfort Educate residents and families on care plans, medications, and health maintenance Coordinate with RNs, physicians, therapists, and other team members for optimal outcomes Uphold Life Care Centers’ patient-first, compassionate care culture Patient assessment Medication administration Wound care Vital signs monitoring Care planning documentation Electronic health records (EHR) IV therapy (per state scope) Infection control Patient and family education Basic life support (BLS) Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active LPN/LVN license in state of practice BLS/CPR certification IV certification (as required by state) Experience in long-term care or rehabilitation preferred
