Job Description:
Provide direct patient care in a home setting, ensuring the comfort and well-being ofpatients- Perform thorough evaluations of patients functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL andIADL performance.- Assess cognitive, psychological and motor aspects of performance and developindividualized intervention plans to achieve favorable outcomes.- Evaluate home environment for hazards or barriers to more independent living.- Identify equipment needs and environmental adaptations resulting in enhancedoutcomes for clients.- Continually assess and revise the occupational therapy care plan, and participate withnursing in the multidisciplinary care plan, as appropriate.- Educate and instruct patients, family members, or other patient representatives, inrehabilitative care and activities necessary to promote the clients health, safety andindependent living.- Accurately document observations, interventions, and evaluations pertaining to patientcare management and services provided. Requirements:
Valid nursing license in the state of California- Minimum 1 year of experience as an LVN in Home Health- Proficient in taking and interpreting vital signs- Full time / Part time position
