Travel Case Management Registered Nurse

As a Travel Case Management RN, you will be responsible for coordinating patient care across multiple settings, ensuring the effective use of resources while maintaining high standards of health care and cost-effectiveness. Assess new patients, review their medical history, and determine the appropriate level of care and treatment plan. Coordinate with healthcare providers to streamline access to necessary medical services from admission through discharge. Procure services for patients such as home health care, medical equipment, and follow-up appointments. Prioritize patient care by continually assessing needs and prioritizing treatments as necessary. Document patient care and progress, securely maintaining comprehensive records. Educate patients and their families about the management of their illness or injury. Act as a patient advocate, understanding and integrating patient preferences in care objectives and treatment plans. Strong clinical skills with a foundation in case management and patient care coordination. Ability to effectively communicate and collaborate with a diverse team of healthcare professionals. Proven skill in critical thinking and problem-solving in dynamic environments. High level of organization and detail orientation. Compassionate patient interaction with a commitment to advocating for patient care quality. Medical Insurance Dental Insurance Vision Insurance Life Insurance Valid registered nurse (RN) license in the state of Oklahoma. Certification in Case Management (CCM) preferred but not required.
