Location:
Brooklyn, Queens, and Nassau County, NY. Field-Based / Onsite Position Summary: As a Hospice Field Admissions Nurse (RN), you will provide compassionate, patient-centered care by performing comprehensive assessments for individuals entering hospice services. In this field-based role, you will evaluate physical and psychosocial needs, develop individualized care plans, and support patients and families during end-of-life care. You will collaborate with a multidisciplinary team to ensure high-quality, culturally sensitive care across home, hospital, and assisted living settings.
Schedule / Shift: Day shift, 40 hours per week.
Key Duties:
- Conduct initial physical and psychosocial assessments for hospice eligibility in home, hospital, and facility settings
- Develop individualized plans of care and recommend appropriate hospice services
- Admit patients into hospice care following NHPCO standards and regulatory guidelines
- Provide education and support to patients, families, and caregivers regarding hospice services
- Engage in community outreach with physicians and providers to support program growth and quality improvement
- Requirements Education:
Required: Graduate from an accredited School of Nursing
Preferred (if applicable): Bachelor of Science in Nursing (BSN)
Licenses & Certifications: Active New York State Registered Nurse (RN) license Valid NYS Driver's License
Skills:
- Strong clinical assessment and care planning skills in hospice or related settings
- Excellent communication and patient/family support skills
- Ability to work independently in a field-based environment
- Proficiency in Microsoft Word and Excel
