Epic Healthy Planet Certification is a MUST HAVERequirement: Epic Healthy Planet Certification (required) Seeking a senior Epic Healthy Planet Analyst with strong population health and value-based care experience.
Required:
- Epic Healthy Planet Certification
- Healthy Planet build and optimization experience
- Registry build and maintenance
- Reporting Workbench and dashboard development
- Population health workflow analysis
- Quality reporting experience (HEDIS, eCQMs)
- Care management and care gap management experience
- Highly Preferred:
- Compass Rose experience
- Risk stratification and predictive analytics
- Patient attribution frameworks
- External data integration (claims, SDOH, HIE feeds)
- Medicare Stars and value-based care programs
- Clarity/Caboodle experience
- Care management tools, care plans, assessments, and outreach workflows
Responsibilities:
- Configure and support Healthy Planet registries, risk models, care gap logic, and attribution workflows
- Develop Reporting Workbench reports, dashboards, and analytics
- Support HEDIS, Medicare Stars, and other quality programs
- Build and optimize care management workflows and tools
- Validate data between Epic, Clarity, Caboodle, and downstream reporting platforms
- Troubleshoot population health reporting and data integrity issues
- Partner with clinical, quality, and population health leadership teams
- Support upgrades, testing, optimization, and end-user support
