Nutrition Educator (Home-Based) – Empowering Seniors to Eat well & Thrive All Seniors Foundation is committed to enhancing seniors’ health through proper nutrition. By providing personalized dietary guidance, we help elderly clients manage chronic conditions, maintain energy, and enjoy a healthier life at home. As a Nutrition Educator (Home-Based), you will meet with seniors to evaluate their nutritional needs, create individualized meal plans, and offer guidance on healthy eating habits. You will collaborate with caregivers and other healthcare professionals to support overall well-being.

Develop meal plans tailored to health conditions, preferences, and cultural factors. Educate patients and families on proper food choices, portion control, and meal preparation. Collaborate with dietitians, nurses, and home aides to ensure consistency of care.

Experience working with elderly populations or in home health environments. Excellent communication and patient education skills.
