About the Role:
We are seeking a Registered Dietitian to join our nutrition team, providing expert guidance and support to our residents, families, and staff. As a Registered Dietitian at Humboldt House, you will be responsible for:
- Conducting nutritional assessments and developing individualized nutrition plans
- Providing medical nutrition therapy to residents with various dietary needs
- Collaborating with our interdisciplinary team to ensure seamless care coordination
- Educating residents and families on healthy eating habits and disease management
- Maintaining accurate records and documentation of resident care
Responsibilities: As a Registered Dietitian at Humboldt House, your responsibilities will include:
- Conducting comprehensive nutritional assessments and developing individualized care plans
- Implementing and monitoring resident nutrition plans, making adjustments as needed
