We are seeking a motivated and compassionate Licensed Vocational Nurse (LVN) to join our home health and hospice team.
Position Summary:
The LVN provides skilled nursing care to adult patients in their homes; coordinates care with the interdisciplinary team and serves as a primary point of contact under the Director of Nursing. This role requires strong clinical judgment, excellent communication skills, and a passion for patient advocacy.
Responsibilities:
Provide skilled nursing care to adult home health and hospice patientsPerform patient assessments and develop individualized care plansCoordinate care with physicians, families, and interdisciplinary team membersEducate patients and caregivers on diagnoses, medications, and care plansMaintain accurate and timely clinical documentationParticipate in on-call rotation and respond to PRN/emergency visits as needed Qualifications Current Texas LVN license (required) Minimum 1 year of home health or hospice experience preferredCurrent CPR certification (required) Valid Texas driver's license, reliable transportation, and auto insuranceAbility to communicate effectively in EnglishHepatitis consent/declination documentationStrong interpersonal and organizational skillsMarketing experience or community outreach interest is a plus Physical & Work Requirements Ability to stand, walk, and lift patients as neededComfortable working in varying home environmentsPotential exposure to blood-borne pathogensFlexible with local travel and scheduling Why Join Us?
