Physical Therapist – Inpatient & Home Health Rehabilitation

Encompass Health seeks a compassionate Physical Therapist to deliver high-quality rehabilitation services across our inpatient and home-based settings. In this role, you will evaluate patients, develop individualized treatment plans, and provide evidence-based therapeutic interventions to restore function, mobility, and independence. You will collaborate closely with an interdisciplinary rehab team, educate patients and families, and document care accurately. Conduct comprehensive physical therapy evaluations and functional assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercises, gait training, and manual therapy interventions Monitor patient progress and adjust plans of care as needed Educate patients and families on home exercise programs and safety Collaborate with an interdisciplinary rehab team (nursing, OT, speech, physicians) Complete accurate, timely documentation in the EMR per regulatory standards Participate in discharge planning and recommendations for equipment or services Ensure compliance with all safety, infection control, and quality standards Contribute to continuous improvement and professional development activities Orthopedic and neurological rehabilitation techniques Functional mobility and gait training Manual therapy skills Therapeutic exercise prescription and progression Balance, fall prevention, and functional training Patient and family education skills EMR/EHR documentation and clinical reasoning Care planning within interdisciplinary rehab teams Knowledge of rehab outcome measures and tests Understanding of insurance and Medicare rehab regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) or equivalent accredited PT degree Basic Life Support (BLS) certification CPR certification Experience with EMR/EHR systems Knowledge of Medicare/insurance rehab documentation standards
