Primary Care Physician - Montgomery, ALWe are seeking a compassionate and patient-focused Primary Care Physician (PCP) to provide comprehensive care to a senior/geriatric patient population. The role offers a collaborative, team-based environment focused on coordinated care, quality outcomes, and an exceptional patient experience.
Key Responsibilities:
Provide comprehensive primary care to senior and geriatric patients.
Evaluate, diagnose, and treat patients according to established standards of care.
Manage a patient panel of approximately 500 or fewer patients.
See approximately 8-13 patients per day.
Participate in team huddles and collaborative care planning.
Maintain continuity of care and a high-quality patient experience.
Complete accurate and timely EMR documentation.
Collaborate with multidisciplinary care teams to support patient outcomes.
Review quality outcomes and medical records with leadership.
Qualifications:
MD or DO from an accredited institution.
Active, unrestricted Alabama medical license.
Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
Active DEA license.
Medicare and Medicaid provider numbers.
