APPLY NOW✨ RN - Nurse Care Manager ✨📍 Fishkill, NY | Hudson Valley/Putnam County💰 $90,000–$105,000 + Benefits🕐 Full-Time | Monday–Friday | 9 AM–5 PM 🚗 Personal vehicle required + mileage reimbursement We’re seeking a Nurse Care Manager to provide clinical oversight and care coordination for individuals with intellectual and developmental disabilities in a community-based residential setting. This is a great opportunity for an RN who enjoys care coordination, clinical assessment, medication oversight, staff training, and working independently across multiple residential locations.
💙What You’ll Do:
- Provide nursing assessments, care coordination, and health oversight for approximately 25–30 individuals
- Travel weekly between residential homes in Brewster, Patterson, Carmel & Pawling
- Develop Plans of Nursing Services (PONS) and maintain accurate clinical/EHR documentation
- Oversee medications, AMAP staff, and pharmacy coordination
- Train and provide clinical guidance to Direct Care staff
- Coordinate medical appointments and collaborate with families, providers, and healthcare teams
- Monitor changes in health status and coordinate follow-up or urgent care as needed
Qualifications:
- Current NYS RN license required
- Nursing degree from an accredited program; Bachelor’s preferred
- Prior nursing experience preferred; ID/DD experience a plus
- Strong assessment, care coordination, documentation, and communication skillsEMR/computer proficiency
- Reliable personal vehicle required
- Comfortable traveling between multiple community residential locations
