Community Health Worker

Community Health Workers at Carle Health serve as trusted partners between patients, families, and care teams. In this role, you will support community members by connecting them to primary care, social services, and preventive health programs. You will conduct outreach, home or community visits, basic health screenings, and education on managing chronic conditions. Working closely with nurses, physicians, and social workers, you help reduce barriers to care, coordinate appointments, and promote patient advocacy. Conduct community outreach and engagement to identify individuals needing health and social support Provide health education and coaching on preventive care and chronic disease management Connect patients with primary care, social services, and community resources Assist with scheduling appointments and coordinating follow-up care Perform basic health screenings within scope of training and protocols Document patient interactions accurately in EHR systems Collaborate with nurses, physicians, and social workers on care plans Advocate for patients by helping reduce barriers such as transportation or access to resources Track and report outreach activities and outcomes Participate in trainings and team meetings to improve community health programs Community outreach Patient education Care coordination Basic health screenings Motivational interviewing Electronic health records (EHR) documentation Data entry and reporting Knowledge of community resources HIPAA and patient privacy compliance Bilingual communication (preferred) Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Community Health Worker (state-recognized, if applicable) CPR/BLS certification Valid driver’s license and reliable transportation
