Registered Dietitian

Education: Bachelor’s degree or an equivalent combination of education and experience

Experience: 1-3 years of experience preferred

Responsibilities: Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodation: Assessment & Care Planning: Evaluates patient nutritional status, lab values, and weight, creating tailored dietary plans based on diagnoses, food preferences, and cultural backgrounds. Home Visits and Education: Conducts in-home counseling for patients and family/caregivers regarding nutrition, food safety, and disease management. Clinical Interventions: Addresses malnutrition risk, manages specialized diets (e.g., renal, diabetic, cardiac, wound healing promotion), and modifies diets for issues like swallowing difficulties. Collaboration and Documentation: Communicates with providers to adjust plans and documents visits according to agency and regulatory requirements. Monitoring and Follow-up: Conducts regular, ongoing assessments to track progress and adjust interventions. Other duties as assigned.
