Emergency Department Care Navigator

Montage Health is seeking an ED Clinical Care Coordinator to act as the gatekeeper of the Emergency Department, providing early visibility of admissions for proactive bed planning and appropriate statusing. You will collaborate with the Care Navigation Hub, ED physicians, nurses, and interdisciplinary teams to optimize patient flow, coordinate dispositions, and ensure timely transitions to inpatient, observation, or outpatient pathways.
