Adaptive Home Health is seeking a Physical Therapist (PT) to manage a full caseload in the Texas field-based model. You will perform in-home assessments, deliver skilled PT visits, and develop a care plan addressing gait, transfers, strengthening, and home safety while ensuring measurable outcomes. You will coordinate with RN/MD and the care team, supervise PTAs when needed, and document thoroughly in HCHB. A Texas PT license and daily travel are required.
