Physical and Occupational Therapist

Essentia Health seeks a licensed Physical/Occupational Therapist to deliver patient-centered, evidence-based rehabilitation services across inpatient and/or outpatient settings. Responsibilities include comprehensive evaluations, developing and progressing individualized treatment plans, documenting in the EMR, and collaborating with an interdisciplinary team. You will educate patients and families, support safe discharge planning, and contribute to quality and process-improvement initiatives. Perform comprehensive physical and occupational therapy evaluations Develop and implement individualized, evidence-based treatment plans Provide therapeutic interventions to improve mobility, function, and independence Document assessments, goals, and progress in the EMR accurately and timely Collaborate with physicians, nurses, and interdisciplinary team members Educate patients and families on conditions, treatment plans, and home programs Participate in discharge planning and recommendations for ongoing services or equipment Contribute to quality improvement and clinical best-practice initiatives Maintain compliance with regulatory, safety, and professional standards Patient assessment and evaluation Developing individualized treatment plans Therapeutic exercise and manual therapy Functional mobility and ADL training Electronic medical record (EMR) documentation Interdisciplinary care coordination Patient and family education Outcome measurement and reporting Home exercise program design Regulatory and safety compliance Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Physical Therapist (PT) or Occupational Therapist (OT) license in state of practice Doctor of Physical Therapy (DPT) or Master’s in Occupational Therapy (MOT/MSOT) BLS/CPR certification Clinical experience in acute, outpatient, or home health settings
