Job Description:
Job Description Silver Haven Home Health LLC is seeking a Part-Time Licensed Vocational Nurse (LVN) to provide skilled nursing follow-up visits to home health patients throughout the Dallas area. This position is ideal for an LVN with home health experience who is looking for a flexible, patient-based schedule.
Duties:
Complete skilled nursing follow-up visits in patients' homes.
Monitor patient condition and report changes to the supervising RN.Provide medication management and patient/caregiver education.
Perform wound care and other ordered skilled nursing treatments within LVN scope.
Follow established plans of care and physician orders.
Complete accurate and timely visit documentation.
Communicate and coordinate with the RN, DON, physicians, and care team.
Requirements:
Current and unrestricted Texas LVN license.
Home health experience preferred.
Current CPR/BLS certification.
Strong clinical assessment and documentation skills.
Valid driver's license, auto insurance, and reliable transportation.
Ability to travel throughout the Dallas service area.
Ability to work independently and complete documentation timely.
