Long-Term Acute Care Registered Nurse (LTAC RN)

Gifted Healthcare seeks an LTAC RN to provide high-acuity, evidence-based care to medically complex patients in a long-term acute care setting. The RN will perform comprehensive assessments, manage ventilators and complex wounds, administer medications, and coordinate multidisciplinary care to optimize outcomes. Deliver comprehensive nursing care to medically complex LTAC patients, including assessments, wound care, ventilator and trach management Administer medications, treatments, and interventions per provider orders and monitor patient response Collaborate with physicians, therapists, and interdisciplinary team to develop and update individualized care plans Monitor and document patient status, vital signs, and clinical changes accurately in the medical record Educate patients and families on conditions, treatments, and discharge plans to support continuity of care Follow infection control, safety, and quality standards to ensure a safe clinical environment Support and mentor other nurses and support staff while promoting a respectful, team-based culture Communicate effectively with staffing coordinators and leadership to optimize scheduling and continuity of care LTAC nursing Acute care nursing Ventilator management Tracheostomy care Wound care IV therapy and medication administration Cardiac and respiratory monitoring Infection control and prevention Patient and family education Electronic Health Records (EHR) documentation Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active, unencumbered RN license in state of practice BLS (Basic Life Support) certification required ACLS (Advanced Cardiovascular Life Support) strongly preferred 1 year recent acute care, step-down, ICU, or LTAC experience preferred Proficiency with electronic health records (EHR) systems Familiarity with ventilator and tracheostomy management Experience with complex wound care and infusion therapy
