Nurse - LPN/LVN, Rehab - Per Diem / PRN - Pay up to $40/HR

Key Responsibilities: Provide basic nursing care such as administering medications, monitoring vital signs, and assisting with mobility exercises tailored to each patient's rehabilitation goals. Document patient progress accurately and report any changes in condition to the supervising healthcare team. Assist patients with daily activities such as bathing, dressing, and eating while promoting independence whenever possible. Educate patients and families on post-discharge care plans and techniques for maintaining progress at home. Collaborate with physical therapists, occupational therapists, and other healthcare professionals to implement comprehensive care plans for rehabilitation patients.

Work Environment: The role is based in rehabilitation centers or hospital rehab units where LPNs/LVNs work closely with patients recovering from surgeries, injuries, or chronic conditions requiring long-term therapy support. This position requires patience, compassion, and physical stamina as it involves assisting patients through challenging recovery processes.
