Job Responsibilities Include but are not limited to: Performing admission assessments and reassessments for all residents in the home Developing, monitoring, and reevaluating plans of care Day to day oversight of personal care aides Interacting with primary care, specialty and hospital providers to ensure resident needs are being met Following all regulatory guidelines Triaging and assessing residents to determine healthcare needs Participating in continuous quality improvement program, quality assurance activities and regular chart audits
