Physical Therapist

FOX Rehabilitation, a clinician-owned geriatric rehab practice, seeks a Physical Therapist to deliver evidence-based, patient-centered care in senior settings. In this role, you will evaluate and treat older adults, create individualized care plans, and collaborate with an interdisciplinary team to restore function, mobility, and independence. You will document outcomes, participate in care planning, and educate patients, families, and caregivers. Evaluate geriatric patients and develop individualized treatment plans Provide evidence-based therapeutic interventions to improve mobility and function Document assessments, progress notes, and outcomes in EMR systems Collaborate with physicians, nurses, and other team members on care plans Educate patients, families, and caregivers on exercises and safety Monitor patient progress and adjust treatment plans as needed Ensure compliance with Medicare and other payer regulations Participate in professional development and clinical mentoring Contribute to a positive, patient-centered team culture Physical therapy evaluation and treatment Geriatric rehabilitation Therapeutic exercise prescription Manual therapy techniques Balance and gait training Functional mobility training Care planning and documentation (EMR) Patient and caregiver education Interdisciplinary team collaboration Knowledge of Medicare and billing compliance Medical Insurance Dental Insurance Vision Insurance 401k Active state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) or equivalent Basic Life Support (BLS) certification Clinical experience with geriatric population Knowledge of Medicare and reimbursement guidelines
