Provide personal and basic nursing care to clients in their homes, following an established plan of care. Assist with hygiene, transfers, mobility, meals and companionship; document services and report changes in clients’ conditions. Work includes light housekeeping and travel within the assigned service area. Provide personal hygiene and daily living care, including bathing, shaving and mouth care. Assist clients with transfers, repositioning and ambulation. Provide basic nursing care and prescribed exercises as instructed by the care team. Prepare meals according to client preferences and special diets when directed. Offer companionship and perform assigned light housekeeping. Document care, report condition changes, and follow the care plan and patient privacy requirements.
