Description:
American Traveler is seeking an experienced RN Case Manager for an acute care Care Coordinator role requiring prior Dignity Health/CommonSpirit experience and an active CA RN license. Details Acute care hospital setting within the Care Coordinator/Case Management unit Uses Cerner EMR8-hour day shifts scheduled 08:30-17:00 Requires working 2 weekends per month13-week assignment with 12 hours of orientation
Requirements:
- Active CA RN license requiredBLS certification requiredLA City Fire & Safety certification required
- Minimum 2 years of RN experience required
- Solid experience in Care Coordination required
- Prior experience with CA Dignity Health or CommonSpirit facilities required
- Copy of driver's license required for consideration
- Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including employment dates and rehire eligibility
Additional Information:
Coordinate patient care and manage case load within an acute care hospital setting Candidates may not have been directly employed by any CommonSpirit-associated facility, including CHI or Dignity Health, within the past year
