Primary Care Physician - Sign on Bonus

About the Role: We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long-term patient relationships in a supportive outpatient environment. You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. Key Responsibilities: Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions. Deliver preventive care, wellness visits, and health screenings with a focus on early intervention. Develop and manage individualized care plans for a predominantly geriatric population. Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care. Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes. Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement. Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices.

Qualifications: MD or DO from an accredited medical school Internal Medicine or Family Medicine or Geriatric Medicine Active, FL license Current DEA registration At least 3 years of experience in Outpatient Primary Care Strong clinical judgment with a focus on preventive and longitudinal care Excellent interpersonal and communication skills Commitment to a team-based, patient-first care model
