In-Home NP/PA Clinician Per Diem & Flexible Travel

Signify Health is seeking a part-time Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, visiting patients at their homes and bridging gaps in care. You will conduct reviews, evaluations and, when needed, diagnostic testing in collaboration with health plans. Requirements include an active, unrestricted license, board certification, and willingness to travel locally within licensed states.
