Credentialing Requirements:- Active Iowa Medical License
- Active DEA (with Iowa address)
- Active CDSBoard Certified in: ABEM, ABFM, or ABIM (or osteopathic equivalents)
- Residency trained in specialtyACLS, ATLS, and PALS required
- If ABEM-certified: must provide current ACLS and one-time proof of ATLSAdditional Documentation Required:
- CME from past 2 years
- Case logs from past 2 years
- Copies of all education & training diplomas
- Social Security CardDriver's License
- Current color photo (to be emailed to MSO)
- Immunization records: TB, Flu, Hep B, MMR, Varicella
