Field-Based RN Case Manager - Home Health in Waco

Alamo Hospice is seeking an RN Case Manager to join our growing team in Waco, Texas. This field-based role manages a patient caseload, delivers therapeutic nursing, and coordinates care for patients and families in home settings. Territory covers Waco, TX and surrounding counties. Schedule is Monday–Friday 8am–5pm with on-call 1-2 weeknights/week and one weekend every 4–5 weeks. Benefit-rich role with a compassionate hospice team.
