After-Hours Hospice Registered Nurse

Provide overnight hospice nursing care across assigned local communities, assessing patients and families, administering medications and treatments, managing symptoms, documenting care, and coordinating with physicians and community partners. Work independently while collaborating with an interdisciplinary team to support patients and families. Deliver professional nursing care throughout the nursing process within scope of practice. Assess patients and families and contribute to individualized care plans. Complete visits as needed to meet overnight patient and family needs. Administer medications and treatments according to physician orders. Document assessments, care, and pain and symptom management outcomes accurately and promptly. Communicate patient condition changes to physicians and coordinate services and equipment with community partners.
