Key Responsibilities: Engage in collaborative care planning with clients and the healthcare team, identifying and implementing interventions that effectively address the patients' rehabilitation and treatment goals. Utilize evidence-based practices and established social work methodologies to conduct assessments and plan care that addresses psychosocial barriers to treatment. Provide supportive counseling services, ensuring patients adjust effectively to treatment modalities, such as dialysis, which directly contributes to improving their quality of life. Deliver educational and goal-directed counseling to individuals pursuing transplant options, guiding them through the emotional and psychological considerations involved. Assist clients in accessing community resources, such as vocational rehabilitation and home health services, thus facilitating optimal treatment outcomes. Maintain current knowledge of local rehabilitation programs, aiding patients in referrals to enable their employment opportunities and educational pursuits. Assess client understanding of advance directives, guiding discussions regarding their preferences and ensuring relevant forms and information are made accessible. Conduct comprehensive psychosocial evaluations that inform treatment plans and enhance overall coordination of care. Patient Population / Client FocusYour counseling practice will primarily focus on patients undergoing rehabilitation for various medical conditions, including those requiring dialysis and transplants. You will work closely with individuals facing significant life changes and challenges, aiming to enhance their psychosocial well-being and empower them to navigate these transitions successfully.

Qualifications: A Master's degree in Psychology, Counseling, Social Work, or a related field. A valid Social Work License, which may include LMHC, LMFT, or LICSW as per state regulations. A clean professional history without any Medical Malpractice Claims, verified by a third party. Authorization to work in the United States.

Preferred Experience: Experience in a clinical setting focusing on rehabilitation or community health services. Proficiency with telehealth systems and digital tools for patient assessment and care. Familiarity with local community resources and supports available for mental health and rehabilitation.

Compensation and Benefits: We offer a competitive salary range of USD 52,000 to 100,000 per year, commensurate with experience.
