Lead Palliative Care Physician

Monogram Health is seeking a full-time Lead Palliative Care Physician to join our team amid continued nationwide growth. In this in-market, field-based role, the physician will care for seriously ill patients in their community, including individuals with CKD/ESRD, COPD, and CHF, primarily delivering care in the home. Working closely with the Market Physician Executive, the clinician will apply clinical expertise in a managed care, population health context to promote palliative care and focus on patient quality of life. Evaluate and treat symptoms that may interfere with quality of life, like pain, pruritis, depression, anxiety, insomnia, etc. Refer members to appropriate specialists to drive clinical care (Monogram-based specialists or locally-aligned specialists) Conduct goals of care and family meetings as needed and document patient/family wishes in state AD/POLST forms Support patients utilizing our IDT, including Palliative Care Chaplain CompensationCompetitive salary plus company bonus program The base salary range for this role is $260,000 to $340,000 Relocation assistance and sign on bonus on case-by-case
