Description:
American Traveler is seeking an RN Care Coordinator (Case Management) for a 13-week assignment requiring an active MA RN license and Epic charting experience. Details Case Management/Utilization Review unit (CCM Nursing) Manages a caseload of patients across the episode of illness from pre-admission to post-discharge Coordinates care prescribed by an interdisciplinary team using patient assessment, care guidelines, protocols, and payer regulations Day shift schedule from 07:00-15:3013-week assignment length Uses Epic EMR for charting
Requirements:
- Active MA RN license required; no pending or conditional licenses accepted
- Both NURSYS and Board of Nursing verification required for licenseAHA-issued certifications only
- Flu vaccine required with no religious or medical exemptions allowed
- Epic charting experience required
Additional Information:
- Participates in ongoing evaluation of practice patterns and systems to support quality, cost, and satisfaction outcomes
- Mobilizes resources to maximize efficiency of care delivery
- Local candidates must reside within 60 miles of the facility to be considered local
- Timekeeping is managed through UKG/Kronos
