Job Title: Physical Therapist

We are looking for a dedicated Physical Therapist to become part of a collaborative outpatient rehabilitation team. This role focuses on helping patients regain mobility, reduce discomfort, and improve overall physical function through personalized treatment programs. Key Responsibilities: Patient Assessment & Treatment
- Conduct comprehensive evaluations to assess patients' physical limitations, mobility, and functional abilities.
- Develop individualized treatment plans based on clinical findings and patient-specific goals.
- Deliver evidence-based physical therapy interventions to promote recovery, reduce pain, and improve overall function.
- Reassess patient progress regularly and adjust treatment plans to maximize outcomes. Patient Education
- Educate patients and family members on prescribed exercises, recovery expectations, injury prevention, and techniques to maintain long-term physical health.
- Encourage active participation in rehabilitation by providing guidance on home exercise programs and self-management strategies. Documentation & Clinical Records
- Maintain complete, accurate, and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries. Care Coordination
- Collaborate with physicians, referring providers, and other members of the healthcare team to coordinate comprehensive patient care.
- Communicate patient progress and recommendations to appropriate healthcare professionals to support continuity of care.
Qualifications: Education
- Doctor of Physical Therapy (DPT) degree from an accredited program. Licensure
- Current and active Florida Physical Therapist license. Skills & Experience
- Strong clinical assessment and treatment planning abilities.

- Ability to work effectively both independently and as part of a multidisciplinary team.
