📍 Location: Bronx, NY – 100% field-based🕘 Schedule: Monday–Friday, 9:00 AM–5:00 PM📅 Weekend Coverage: Some weekend coverage required💰 Compensation: Based on experience🎓 Tuition repayment + amazing benefitsThis role provides psychosocial support to hospice patients and their families across home care, nursing home, and inpatient settings. The Social Worker will be an integral member of the interdisciplinary hospice team, developing individualized plans of care, supporting patients and families through the hospice and bereavement process, and coordinating with clinical and facility staff.

Requirements: NYS LMSW or LCSW – requiredMSW from an accredited school of social workHealthcare social work experience preferredOpen to new graduates with hospice/palliative care internship experienceStrong communication, organization, and listening skillsComfortable working independently in a field-based roleAbility to work effectively as part of an interdisciplinary team
